PLEASE READ THIS DOCUMENT CAREFULLY BEFORE SIGNING. THIS IS A LEGALLY BINDING DOCUMENT BETWEEN THE PARENT/GUARDIAN AND KIDS INNOVATIVE LTD (KIDS INNOVATIVE). THIS FULLY SIGNED FORM MUST BE SUBMITTED BY A PARENT OR LEGAL GUARDIAN BEFORE ANY CHILD IS ALLOWED TO PARTICIPATE IN THE AFTER SCHOOL PROGRAMS OR CAMPS.

Parent/Guardian Agreement for Kids Innovative After School Programs and / or Summer Camps.

I, the undersigned, agree to the following terms and conditions for my Child's participation in the Kids Innovative after school program:

• Acknowledgement of Risks: I understand that my Child may be exposed to physical injury, economic loss, and property damage during the camp. I acknowledge and accept these risks.

• Health and Safety: I confirm that my Child is fit to participate in the camp and that I will seek medical advice if necessary before the program begins.

• Liability Release: I release Kids Innovative and all associated personnel including Kids Innovative administration, staff and volunteers from any liability for injuries or losses sustained by my Child during the camp.

• Indemnity: I agree to indemnify Kids Innovative against any claims or liabilities arising from my Child's participation in the after school program.

• Medical Authorization: In case of emergency, I authorize Kids Innovative to obtain medical treatment for my Child. I will bear all expenses arising from such treatment.

• Behavioural Expectations: My Child will follow all instructions from Kids Innovative staff. Kids Innovative reserves the right to send home any child whose behaviour is deemed dangerous or unmanageable.

• Refund Policy: I understand that there are no refunds after 24 hours prior to the after school program start date.

• Image Use: I agree/disagree to allow Kids Innovative to use my Child's image for promotional purposes. I release Kids Innovative from any liability arising from the use of my Child's image.
